
What is support Coordination?
Direct Care Australia is registered to provide all three levels of support coordination.
If your plan has allocated funds for support coordination Direct Care Australia can help bring your NDIS Plan to life.
With so many NDIS options and service providers available, it can be difficult to decide which services are right for you – and how to connect to them. This is where we can help. We support you to put your NDIS plan into action, using your choice of services and providers. As Support Coordinators, we are required to provide you with impartial advice.
There are three levels of support coordination that can be included in your plan

Support Connection
This support is to build your ability to connect with informal, community and funded supports enabling you to get the most out of your plan and achieve your goals.

Support coordination – coordination of supports
This support will assist you to build the skills you need to understand, implement and use your plan. A support coordinator will work with you to ensure a mix of supports are used to increase your capacity to maintain relationships, manage service delivery tasks, live more independently and be included in your community.

Specialist support coordination
higher level of support coordination, specifically targeted to people whose situations are more complex and who need specialist support. A specialist Support Coordinator will assist you to manage challenges in your support environment and ensuring consistent delivery of service.

Direct Care Australia’s Support Coordination team can help with:
Managing NDIS budgets to maximise your plan.
Liaising with the NDIA to obtain the supports and funding you require. This includes lodging plan reviews when the amount of funding is incorrect or insufficient in your current plan.
Select services and service providers to realise your plan, include contract initiation, review and monitoring.
Obtain new specialised and customised equipment, or repair old equipment using NDIS funding.
Attend meetings with you to provide support and/or advocacy where you request.
Listen to your needs and preferences and seek your consent before putting in place any service.
